I’ve always felt “adequate” at IVs, but any hard sticks would typically be too much for me. One of the reasons I’m so excited about working in Endoscopy is a chance to get REALLY GOOD at IV starts. When I’m working as an intake nurse, I end up starting 5+ IVs in a day! In the past 3 weeks of orientation, I’ve come to realize that if I can see and feel it, I can hit it. When I can feel it but can’t see it (or vice versa), that’s when I struggle. I’m trying to watch and learn all the IV tricks from my co-workers, but feel free to leave any advice in the comments below! 👇🏼 I’m probably not the only one who could use some IV tips for difficult sticks! ❤️

Last week a 9yo MN Bichon Frise had presented with a two-week history of gross haematuria (bloody urine). The dog was otherwise healthy and well. The urine analysis hadn’t identified any signs of bacteria, crystals or neoplastic cells to explain his clinical presentation. Abdominal imaging had also failed to identify any obvious causes (e.g. bladder stones or mass lesions) affecting his upper or lower urinary tract including the kidneys, ureters or bladder.
I was called in to perform a urethrocystoscopy to assess the urethra, bladder and ureteric openings using a 3.0mm fiberoptic flexible cystoscope. This was an extremely challenging case due to the huge volumes of blood absorbing all the light being illuminated into the bladder. After extensive lavaging, fresh blood was eventually visualised being ejected into the bladder from the right ureterovesicular junction. This most likely represents a condition called idiopathic haematuria, and is often considered a ‘diagnosis of exclusion’. There are no breed, age, or sex predilections. Many patients will have hematuria as their only symptom; however, urinary obstruction, lower urinary signs from blood clots or systemic signs of anemia can also occur. Colleagues at @amcny have also used Yunnan Baiyao (a haemastatic agent used during wartime) with a reported success rate of 10-15% in confirmed cases.
This dog was started on an ACE-I in attempt to reduce intrarenal pressure and assess his response over the next 7-10 days. If the bleeding continues, then interventional sclerotherapy will likely be the next step in managing this case. Very interesting case!

🔥Burnout tip: arrive early.
You don’t need that gut-twisting, panic sensation of always being late, get it out of your life!
There’s lots of tricks you can try:
•setting multiple alarms
•laying out your clothes the night before
•if it’s somewhere you’ve never been before, scope it out days in advance.
•set the appointment in your phone for 15 minutes earlier to allow for changes in traffic and finding parking.
Once you make a habit of being early, being on time will feel late. 😄 Worst case scenario, you’ll just kill some time in your car before heading in. 🙌🏼
Orientation starts tomorrow so I went on a little drive to check out where we’ll be meeting. Isn’t it beautiful (This is just for day 1, we meet on the hospital campus the rest of the week.) This gave me a chance to see how long it takes to drive there from my house, find the employee parking, and get rid of some of the first day jitters by feeling prepared. 👍🏼
