I’m jumping in on the convo started by @drreneeparo encouraging #dualphysician families to share how they are #reconcilingmedicine while #marriedtomedicine

Neither me or my hubby have a 9-5 job, so it often feels like a dual-doc lifestyle!

My husband is a tennis instructor. He works evenings and weekends. This month, we will celebrate 15 years of marriage. We have been together 22 years! He had been through it all. He is the reason I can “do it all”! But it isn’t and hasn’t been an easy road.

He proposed a couple months after I got accepted to medical school. I moved from LA to San Diego. He stayed in LA to work. We got married a year later and lived 2 hours apart our first 3 years of marriage. He came to San Diego most Friday nights, and got up at 4 am every Sunday to drive back to LA and work.

Fast forward 3 years: I came back to LA for residency. We got our first place together. He was “wife-less” every third to fifth night during my 6 years of residency. 
My work hours got a little easier when I started practice - but our girls came along, completed our family, and added a layer of complexity to the juggle.

Let’s just say I couldn’t do any of it without him. He does the morning routine most days. He works late most nights so I get home before he does, unless a long OR day derails my evening - in which case thank the lord for our god-sent babysitter who stays with the girls until one of us is home!

We share the roles of grocery shopping, laundry (mostly him), cooking (me or nobody- usually nobody), and the girl’s bedtime routine. I handle anything to do with school, birthday parties, planning and booking vacations, while he manages anything to do with home maintenance. 
He is not a doc but understands the unpredictability of my schedule. He is supportive and I am appreciative. I have stopped complaining about his horrible hours - worse than mine in a lot of ways. We make it work. We’re doing life. We’re raising our kids. We are happy.

Who knows what I’m pointing to?

Indocyanine green (ICG) is a fluorescent dye that can be injected intravenously during surgery to highlight blood flow to the bowel! It’s one of my favorite surgical “toys” - good blood flow is one of the key factors in healing when bowel (small or large bowel) is removed and reconnected. The traditional way to check this is to look at the color of the bowel. If it looks pink it’s healthy, if it is starting to get purple or dark, the blood flow may be poor. You can imagine how subjective this can be.

ICG can show a clear demarcation of where the blood is flowing and where it is not. It helps me decide which parts are healthy for reconnection to minimize the chance of poor healing, infection, leak of the connection, or narrowing of the connection down the road! I’ve used it for other purposes too, like tattooing a tumor so I can find it surgically, or to help find the ureters (tubes draining the urine to the bladder) in a mess of inflammation. Surgeons are using it in gallbladder surgery and many new applications are being developed! Love how medicine and surgery continues to evolve to improve care of patients- people think it’s already perfect or it should be in the 21st century! We are working on it!

Reflecting on OWNERSHIP. 
If there is one piece of advice I would give a surgical intern or any intern for that matter, it is to take ownership of the patients you care for.

As medicine changes to become more humane to those delivering care, it should not be lost on any of us who care for patients, that we are, at the end of the day, responsible for the health and outcomes of our patients.

This concept was something engrained in my teaching early in my career. In surgery, there’s a rule, “I cut you, you are my mine”. This mentality is taught early on in training in order to instill a sense of responsibility for the health of those who entrust us with their lives.

When I was a resident, this meant that if a trauma patient went to the ICU in critical condition, I sat by their room, checked on them many times throughout the night and updated my attending of any changes in their condition, EVEN, if another resident was covering the ICU. That patient was mine. It meant that if I went home post-call after I operated on someone, I woke up after my nap, and checked to see how they were doing. I did this because I cared and because I took ownership of my patients.

Today, as an attending surgeon, when a patient establishes care with me, when I operate on them, they are my responsibility. There are some things you can’t check out of - last week while away on vacay I called a patient to give them the biopsy results on the cancer I had found during a colonoscopy. Was something gonna change in their cancer if it waited 2 days for me to come back? No. But I knew they were worried and waiting for the call.

As medicine continues to evolve to improve the quality of life for doctors, transitions in care such as hand offs, sign outs, days off, post-call, should not effect parient outcomes. If they do, we are failing our patients.

I truly believe that if everyone takes a little ownership for the oath we all took when we entered this profession, healthcare delivery can be at its ULTIMATE BEST with a system that protects both doctors and patients.

So, #dearintern, just trust me on this one.
