 #womeninmedicinemonth2018 ▪️I graduated 10 years ago, started my training at Guys and St Thomas’ London - qualifying as a GP in 2013. I started med school at age 18 and as maturity, adult identity and values took shape, my understanding of my life experience helped to clarify and refine who I was and what I wanted to give. It has been life changing, the things seen, learned and the people we care for. Setbacks are natural and I’ve had to let go of niggles of self-doubt during anti-social hours and tough rotations. The camaraderie is second to none, during exam prep, debriefing and consequent friendships made. We take on a lot of responsibility and give a lot of ourselves so it’s important to be insightful, seek support and talk to each other. ▪️I currently work part time in a GP practice with adhoc locum work. I am starting soon as a Medical Student Mentor and on the Executive Committee of a medical indemnity company. I have done a bit of writing for Pulse Magazine historically and help with marketing and press work for my husband’s companies. Portfolio is the way whilst my children are young.
▪️Morale and mental health throughout the phases of medicine can be challenging due to pressures, long hours caring for the sick, the health care system or if going through a hard time ourselves. I am lucky to have made some wonderful #womeninmedicine connection in my life and now on IG where not only are you busy doctor-ing some of you have remarkable talents with cooking, on TV, sports/yoga, writing & business 🙌🏽

🎓Taking on my hard earned Hippocratic oath. Whilst I don’t think I have had any “haters” it wasn’t always smooth and pain free to get to this point. Teenager to young woman, having to let go of niggling self-doubt, the decisions I made and wondering how it was all going to translate into a career. Would I ever find my niche of contentment? (I wanted to be a surgeon in medical school.) Asides failure, my biggest fear was what if I wasn’t meant to be a Doctor? Personal and parental sacrifice. Misspent youth, all for nothing? Luckily time and life experience changed my outlook, clarifying and refining what I wanted to achieve as I grew up.
🎓This photo marked the beginning of the most important chapter of my life. It’s been 15 years since the start of med school, a decade since graduation - 5 years a GP. What would I tell myself at 18, 23 and now? Naive determination saw me through. It’ll be life changing, a tough road, the things I’ve learned, the people’s lives we cross paths with. Our patients know so little of us but yet they trust us with so much of them. Would I do med school again - adapting to responsibility was tough - but do I regret it? Never! Even when the bigger picture didn’t seem real. 🎓
Starting out in medicine at age 18, you have a long way to grow, building adult identity and values, to prepare for the next step. My advice to med students, find YOUR positive balance. Consolidate in study groups, especially with practical exams, share resources and always round the day/week with fun & self-care. I kept perspective through my now lifelong friendships, who I shared the study and fun with; Uni hockey, medics sports teams and tournaments, social event planning; charity fashion show, fun runs, and loved to dance that night away.

I wouldn’t have known it then, coming to the realisation when I knew I was doing something that I truly believed in. So to relinquish the doubt of my youth through the words of Oprah; “The biggest adventure you can ever take is to live the life of your dreams.” keep dreaming 💫 #whitecoatdreams

Meet another member of the team! Working alongside founder and specialist sourdough baker @vanessakimbell is a team that helps make The Sourdough School what it is today.

This is Dr Amrita Vijay, our in-house researcher who works closely with Vanessa to validate research and evidence used in The Sourdough School and also brings her knowledge and perspective to the Royal College of General Practitioners courses. Amrita was also absolutely integral to making sure the research used in Vanessa's latest book was correctly interpreted and applied.

With a shared passion for the science behind sourdough fermentation, and its application to digestibility and supporting the gut microbiota, Amrita also manages and is helping to grow our research database. All Sourdough School students gain access to almost 300 research papers containing studies on the health benefits of sourdough, the microbes involved in the fermentation process, and the gut microbiome.

Did you know that 20% of the UK population is reportedly IBS?

One of the key things that we teach here at the @sourdoughschool is not just how to make sourdough bread but also evidence-based tips and techniques to improve the digestibility and nutrition of bread. This is something especially important for the students who join us who have digestive issues themselves or if they are baking for customers or family who face bread digestion issues.

IBS is one of the main issues that people face when it comes to bread digestion. FODMAP (fermentable oligosaccharides, disaccharides, monosaccharides and polyols) are types of carbohydrate and the malabsorption of these is related to lack of enzymes and the sugars go down to the colon where they ferment causing gas and very unpleasant symptoms. The good news is, the long slow fermentation process of sourdough very much diminishes the level of FODMAP carbohydrates up to 90%. This means sourdough is far more digestible for people with IBS.

We go far beyond teaching simply how to make bread. We teach you why fermentation matters and how the mechanisms by which the fermentation are transformative and help contribute to ease of digestion and better nutritional value. Anybody that has walked into The Sourdough School will know that there is an instant feeling of being at home. There’s a sense of warmth exuding from the eclectic collection of very personal belongings of someone that is deeply passionate about their subject. Although at first glance it might seem that this is a random collection, you soon realise that every single item in here has a ‘raison d'être’, a reason to be, including the posters on the walls. The posters have history, they have age, they have those muted gorgeous faded colours of being used for years and years as a teaching aid.

Now, the posters are used in the classes here at The Sourdough School as a visual aid to convey knowledge and understanding of every aspect of fermentation. @vanessakimbell stands beneath them and teaches about everything from the grain to the soil and the rain to the microbes and ultimately how the growing conditions and the choice of grain affect the end product: the bread.

There’s something really lovely about the way everything in the room has a purpose. There’s a sort of calmness in that. Not being able to get an explanation is frustrating which is why we now have a resident GP. After my family and children, one of the things I feel grateful for is the privilege and trust that people place in me as a Doctor, putting their health, concerns whatever they need in my hands. My work is a strong part of my identity and I have missed it. As they say life swings in roundabouts and when I’m back in a little over a month, I’ll miss the girls more! But the valuable lessons I’m learning in both worlds are with me each and everyday.

A topic close to my heart and the reason I joined IG in January was that after having children my disciplined approach to lifestyle was challenged. The adaptive process to preserve physical and mental wellbeing, motherhood and design a lifestyle to suit our family needs. A shift in my identity compounded by being off work, yet having to maintain my career remotely with CPD is not easy. All whilst looking for a good night sleep and feeling the guilt of compromise or isolation

This is what has developed my interest in #lifestylemedicine , not seemingly complicated in its theories but the discipline to apply it effectively in our own life context can be challenging logistically. Our neurological capacity to form and maintain the habits can be a hurdle

SUNSHINE & MOOD ☀️ #lifestylemedicine ☀️Compared to our ancestors we are spending less and less time outdoors and more under artificial light. Society is considered “light deprived”. Some interesting evidence compiled by S.N. Young [1] explores the link between sunlight, exercise and serotonin production. Serotonin has multiple roles within the body, famously known as a “feel good” neurotransmitter. The more daylight hours we are exposed to the more serotonin we produce. Higher levels of 5-HIAA - (a serotonin metabolite) was detected in the venous blood of people exposed to high levels of daylight independent of the season. [2] .
🧠 Furthering the link between light exposure and serotonin production; a post-mortem study showed an increased amount of brain serotonin in people who died in the summer versus winter. [3]

🏃🏽‍♀️ Exercise has found to have a beneficial effects to helping mood but the length type, intensity and duration not determined. [4]There is also a link between exercise and increased serotonin production. [5]. Again, 5-HIAA the serotonin metabolite is found in higher concentrations in the brain after aerobic exercise; specifically around the cerebral cortex and hippocampus. [6] The hippocampus is involved in our long term memory formation and emotional regulation.

📝 If you have non-seasonal depression the evidence for light therapy is unproven and is not advocated by NICE. A meta analysis of randomized controlled trials [7] concluded the evidence was weak due to study bias and participant reporting. Dr’s are not recommending it in replacement of medication.

☀️ Serotonin pathways are active alongside many others , ie: vitamin D and Melatonin production are linked to light exposure. Vitamin D has many roles and melatonin as we know is linked to sleep. .
🔬 So here is my simplified version of why exercising in the sun can give us a boost and make us feel good. So if you usually work out in a gym, under low light levels you may feel more glorious if you opt to exercise outdoors instead ☀️ 🏃🏽‍♀️ .
Stay hydrated 💦 high SPF, UVA and UVB suncream. Avoid 11am - 3pm sun.

We don’t think there has been a single class ever taught in the school that has not had at least one student who has issues with digesting bread. The more we research the topic, the more we can pass on to our bakers.

Today, The Sourdough School excels in teaching our bakers about the digestibility and potential health benefits of Sourdough. Recently the internet has exploded with information on FODMAPs. It sounds more of a headache than it is. FODMAPs are fructans that have been grouped into a large family of dietary carbohydrates and often spoken about in relation to IBS.

Part of an article written by Vanessa explains studies on sourdough rye showed that the low-FODMAP rye bread caused less fermentation in the colon, and less flatulence, less abdominal pain, less cramps and less stomach rumbling than regular rye bread.
