First day back from reading break and we had class in the phys med room. It was another day of review. Dr. Nik asked us if we had any of the physical testing that we wanted to go over and we covered them and tried them on each other again
We, again, practiced the six deep tendon reflexes:
Bicep – C5
Brachioradialis – C6
Tricep – C7
Semitendinosus – L4
Patellar – L5
Achilles – S1
Next we went through a palpation (method of feeling with fingers or hands during a physical examination) checklist, including:
SPs (spinous processes) and TPs (transverse processes) T1-L5
Pectoralis minor and major
Finally Dr. Nik showed us several cervical stretching and massaging techniques to practise on each other to help each other out during the upcoming finals season. I got lots of practise today on a colleague and my guinea pig husband 😇
Today’s class started with quiz number fourteen! After the quiz, we had a lecture on the Nitrogen cycle and an intro to amino acids
The Nitrogen cycle is the global circulation of nitrogen from the environment to living organisms and back to the environment. It is composed of five steps:
1. Nitrogen fixation – the conversion of gaseous nitrogen to ammonia (NH3)
2. Nitrification – the conversion of ammonia or ammonium (NH4+ formed when water reacts with ammonia) to nitrate (NO3-)
3. Assimilation – the absorption of nitrate, ammonia, or ammonium by plant roots (and animals) and the incorporation of the nitrogen into plant proteins and nucleic acids
4. Ammonification – the conversion of biological nitrogen compounds into ammonia and ammonium through waste (urine)
5. Denitrification – the reduction of nitrate to gaseous nitrogen via bacteria
Finally we had an overview of the amino acids (monomers that combine to form proteins)
There are 20 amino acids: 10 are nonpolar, 5 are neutral/polar, 2 are acidic, and 3 are basic
Of the 20 amino acids, 10 are essential amino acids (EAAs) which mean that the body cannot synthesize them and therefore needs to be ingested through food

Continuing on our #NeurodynamicSeries, the CROSSOVER effect is a proposed theory that during neurodynamic maneuvers (i.e., SLR & SLUMP), a ⬇️ of radicular symptoms through the contralateral side of the spinal cord occur. This is due to a downward ⬇️pull on the spinal cord that can reduce compression/tension on an affected nerve root in a disc protrusion, therefore RELIEVE symptoms😊
Some interesting research has surfaced that shows neurodynamic maneuvers actually REDUCE contralateral nerve root and dural tension.
Rade et al. (2014), studied the effects of spinal cord movements under MRI when performing a unilateral SLR and the results are rather surprising. The results demonstrated that the spinal cord moves caudally during a unilateral SLR, with a 60° SLR, and 2X as much during a bilateral SLR!!! 😲
Shacklock et al (2016) then used similar methods to investigate SLUMP neurodynamic testing and had similar results. Asymptomatic subjects performed a SLUMP test, and then extended the contralateral knee to caudally move the spinal cord. Knee extension led to reduced posterior thigh stretch response on the contralateral side. The proposed mechanism of reduced stretch response on the contralateral side (caudal movement of the neural tissue and dura) was then confirmed in cadavers!😮
Additionally, the crossover mechanism has been shown to ⬇️ symptoms, where caudal displacement of the spinal cord was confirmed radiographically, supposedly reducing tension/compression on the nerve root by the patient’s disc protrusion.
Therefore, this crossover mechanism appears to be a viable method of relieving lumbar radicular symptoms. We will be interested to see how continued research will either help support or deny these findings. In the meantime, the take home message is that if you encounter a crossover effect in an appropriate clinical presentation (i.e disc protrusion) you may want to take advantage of it!! 🤓

la cicatrización lleva 3 fases a saber:
🔷La cicatriz siempre quedará con un textura y características distintas al tejido lesionado. es por esto que en la mayoría de los casos queda adherida o pegada impidiendo que el tejido tenga una correcta movilidad o deslizamiento provocando así una alteración en la función.
🔷Es por esto que al enfrentar este tipo de alteración es recomendable realizar un masaje profundo en la cicatriz o como se conoce comúnmente masaje transverso profundo o técnica de cyriax para así romper las adherencias ocasionadas en el tejido y recuperar su movilidad normal.

A hip condition treated at our practice: Hip labral tear.
A hip labral tear involves the ring of cartilage (labrum) that follows the outside rim of the socket of your hip joint. In addition to cushioning the hip joint, the labrum acts like a rubber seal or gasket to help hold the ball at the top of your thighbone securely within your hip socket.
Athletes who participate in such sports as ice hockey, soccer, football, golf and ballet are at higher risk of developing a hip labral tear. Structural abnormalities of the hip also can lead to a hip labral tear.
* A locking, clicking or catching sensation in your hip joint.
* Pain in your hip or groin.
* Stiffness or limited range of motion in your hip joint.
